APPLICATION OF GLUTEUS MAXIMUS MYOCUTPNEOUS FLAP IN SURGICAL TREATMENT OF SACRAL SORE

Abstract
For the patient prone to the pressure sore from quadriplegia and paraplegia owing to the car accidents, industrial injuries or various intoxication causing consciousness loss, we must attach importance to prevention rather than treatment.

However ,as all paraplegics cannot be persuaded and educated to live within the severe limitations imposed upon them by their condition, pressure sores will occur and they will require repair.

The methods that can completely solve ths various problems of pressrue sore which easily recur to these plegic patient in unsettled yet. In the early step of the treatment for pressure sore, simple skin graft or local skin flap was used. But there was many problems; the pressure sore recurred easily, or the skin flap could not be made large to enough, etc. In the next method, muscle transposition simultaneously with skin graft had enough cushion effect on the site of pressure sore, but this method also had many complicated problems for example, defect of blood supply and skin coverage. So today by making use of gluteus maximus myocutameous flap, the defects of traditional methods wrer made up for. Though as a usual method in treatment of sacral sore, gluteus maximus muscle with its skin and subcutaneous tissue was used as rotation flap, this method is difficult in operative technique and has a problem in donor site repair. So recently transforming this method into V-Y advancement flap or island transposition flap, the operation can be performed easily.

Authors performed operations for 11 cases of sacral sore using rotational GMMC flap in 5 cases, V-Y advancement GM MC flap in 5 cases and island transposition flap in 1 cases, we analyse merit and defects of each method and report them with the review of the articles.
